I certify that I am the legal owner or authorized custodian of the pet listed in this form and that the information I provide is true, complete, and current to the best of my knowledge.
I understand that grooming involves inherent risks, including but not limited to nicks, cuts, clipper irritation, nail quicking, stress, anxiety, skin irritation, exposure of pre-existing conditions, and complications related to age, health, behavior, coat condition, or handling sensitivity.
I understand that grooming may expose or aggravate hidden or pre-existing conditions, especially in elderly pets, medically fragile pets, anxious pets, aggressive pets, or pets with heavily matted coats, skin issues, injuries, mobility limitations, or behavioral sensitivities.
If my pet’s coat is matted, pelted, compacted, or unsafe to brush out, I authorize Simon Says Groom to use professional judgment to perform a humane shave-down or coat removal necessary for my pet’s safety and comfort. I understand this may reveal skin irritation, sores, bruising, hotspots, parasites, thin areas, or other pre-existing conditions that were hidden by the coat.
I understand that extra charges may apply for severe matting, de-matting time, parasite treatment, special handling, late pickup, excessive coat work, long coat maintenance, double coats, or behavior that makes grooming more difficult or time-consuming.
I understand that Simon Says Groom may refuse, shorten, modify, or stop any service if continuing would be unsafe or inhumane for my pet or for staff. This includes but is not limited to aggression, repeated escape attempts, panic, pain response, inability to safely stand, medical fragility, or severe coat condition.
If fleas, ticks, or other parasites are discovered, I authorize Simon Says Groom to take reasonable sanitation and treatment-related steps necessary to protect the pet, the staff, and the grooming environment. Additional fees may apply.
If Simon Says Groom reasonably believes veterinary attention is needed during or after services and I cannot be reached promptly, I authorize Simon Says Groom to obtain veterinary care for my pet. I understand that my listed veterinarian may be contacted first, but another licensed veterinarian or emergency clinic may be used if necessary. I accept responsibility for resulting costs unless otherwise required by law.
I understand that Simon Says Groom is not responsible for conditions that are pre-existing, hidden by coat condition, or reasonably related to disclosed grooming risks when ordinary care is used.
I agree that this agreement may be completed and signed electronically and that my typed name, submitted form, and stored electronic record may be used as evidence of my agreement and consent.
By signing below, I confirm that I have read this agreement, understand it, had the opportunity to review it before submitting, and voluntarily agree to its terms.
